Variolation was the method of inoculation first used to immunize individuals against smallpox (Variola) with material taken from a patient or a recently variolated individual, in the hope that a mild, but protective, infection would result.

The procedure was most commonly carried out by inserting/rubbing powdered smallpox scabs or fluid from pustules into superficial scratches made in the skin. The virus was normally spread through the air, infecting first the mouth, nose, or respiratory tract, before spreading throughout the body via the lymphatic system. In contrast, infection of the skin usually led to a milder, localized infection - but, crucially, still induced immunity to the virus. The patient would develop pustules like those caused by naturally acquired smallpox. Eventually, after about two to four weeks, these symptoms would subside, indicating successful recovery and immunity.

The method was first used in China, India, parts of Africa and the Middle East before it was introduced into England and North America in the 1720s in the face of some opposition.

Virus collectives from the wounds of smallpox patients who had survived the disease were used. Usually, material was taken from a pustule of sick people using a lancet, which was then introduced subcutaneously, i.e. through small wounds, into the arm or leg of healthy people.<2> As a result, healthy people were administered attenuated (weakened) viruses. A live attenuated vaccine was used. The immune response should be triggered by the multiplication of the viruses. However, the viruses could become disease-causing again through reverse mutation during their replication, which led to a relatively high death rate (2–3%)<3> among those treated in this way. After all, at that time the mortality rate with this treatment was lower than after infection without previous variolation.<4> <5> According to a calculation from 1722, this was 1:14 for non-inoculated and 1:91 for inoculated children.<1> Another problem was that other diseases, such as g. syphilis or tuberculosis.<2> In addition, a (real) smallpox epidemic could be triggered by the variolation.<6>

Looking at history and origins of  vaccination, this historical view somewhat reminds me of today.
